I, the undersigned parent or legal guardian of the above-named minor participant, acknowledge that participation in youth flag football and related tournament activities involves inherent risks. These risks may include, but are not limited to physical contact with other participants, falls, collisions, contact with equipment or playing surfaces, weather conditions, and other risks associated with athletic competition. I understand these risks may result in injury including sprains, fractures, concussions, or other serious injuries.
I voluntarily allow my child to participate and assume all risks of injury, illness, or damages that may occur while participating in Game Time Sports League tournaments or related activities including warm-ups, games, or presence at the event location.
In consideration of being allowed to participate, I hereby release, waive, discharge, and agree not to hold liable Game Time Sports League, including its owners, directors, staff members, coaches, volunteers, referees, event organizers, sponsors, field operators, and affiliates from any and all liability, claims, demands, or causes of action arising from injury, loss, or damages sustained while participating in the tournament or attending the event. This applies to the fullest extent permitted by law.
I certify that my child is physically able to participate in athletic activities. I understand that Game Time Sports League does not provide medical insurance coverage for participants and I accept full financial responsibility for any medical treatment resulting from injury. In the event of an emergency, I authorize tournament staff to seek reasonable medical treatment for my child if I cannot be immediately reached.
Participation in this event is voluntary. At any time that my child or I feel unsure, insecure, uncomfortable, or unsafe, we understand we may stop participation and remove ourselves from the situation immediately.
I acknowledge that concussions and other injuries can occur during sports participation. If a participant shows signs of injury or concussion, tournament officials may remove the athlete from play for safety reasons and medical clearance may be required before returning.
I agree that my child, family members, and team representatives will behave respectfully and demonstrate good sportsmanship. Game Time Sports League reserves the right to remove participants or spectators engaging in unsafe or disruptive behavior.
I grant permission to Game Time Sports League to use photographs or video recordings taken during the tournament that may include my child for promotional or social media purposes without compensation.
If any part of this waiver is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
By acknowleding and finishing this registration process, I confirm that I am the parent or legal guardian of the participant listed above, I have read and understand this waiver, and I voluntarily agree to all terms contained in this document.
